Dementia and 24-Hour Home Care in Rancho Bernardo
When a parent or spouse in Rancho Bernardo is living with Alzheimer's or another form of dementia, staying in the home they know can often still work. Heritage Senior Care provides non-medical in-home care, from a 4-hour visit to long shifts of 8 to 24 hours a day, in Seven Oaks and Oaks North, Westwood, Bernardo Heights, the golf course neighborhoods around the Rancho Bernardo Inn, and the hillside streets of High Country West and The Trails. We have been family-owned since 1983, and all scheduling is handled from our one office in Carlsbad.

Planning dementia care around Rancho Bernardo homes
Rancho Bernardo was planned from the early 1960s as a set of distinct neighborhoods, many with their own homeowners association and recreation center, and many residents have lived here since they retired. Each kind of home changes what dementia care looks like day to day:
Seven Oaks and Oaks North: two of Rancho Bernardo's long-established 55+ communities, with single-story homes, twin homes and condominiums built from the 1960s through the 1980s. Single-level living helps, but step-down living rooms, raised thresholds, older bathrooms and patio steps still need planning.
Golf course neighborhoods: homes near the Rancho Bernardo Inn course, Oaks North Golf Course and the Bernardo Heights course, where fairways, cart paths and greenbelts run right behind the back fence.
Westwood and Bernardo Heights: larger family neighborhoods on both sides of I-15, with many two-story plans. If the home has two stories, the stairs and the location of the bedroom and bathroom are usually the first things we plan around.
High Country West, The Trails and the hillside streets: homes on curving streets and larger lots, some with steep driveways, slopes and long walks to the nearest neighbor.
Condominiums and townhomes: communities near Bernardo Center Drive, Rancho Bernardo Road and Pomerado Road, where shared entries, carports, upstairs units and guest parking all shape the day.
At the free assessment we walk through the home with you, including stairs, bathrooms, entry doors, patios, garages, gates and yards, and plan care around how your loved one actually spends the day.
Long shifts: 8 to 24 hours a day
As dementia progresses, a short visit rarely covers the hours families worry about most: the long afternoon, the evening and the night. Some families begin with 4-hour visits a few days a week and add time as needs change. We schedule:
Full-day shifts of 8 to 12 hours, so one caregiver can carry the day from breakfast through dinner, including personal care, meals and activities
Overnight shifts, so someone is awake in the home if your loved one gets up, feels confused or needs help to the bathroom
24-hour care, with caregivers scheduled in shifts around the clock so the home is never without help
Heritage provides 24-hour care through scheduled shifts. See how our 24-hour and overnight care works, or read what to expect from 24-hour dementia care at home.
Keeping familiar routines
A steady rhythm to the day helps reduce confusion and anxiety. Our caregivers keep routines going for as long as they are safe and enjoyable, whether that is a walk along the greenbelt, coffee on the patio, a class or lunch at the community center, the Friday farmers market at Bernardo Winery, a drive past the golf course or time in the garden. Rancho Bernardo is built around the car, so for many people here, a ride to a familiar place is a big part of the day. Our caregivers also help with:
Personal care: bathing, dressing, grooming, toileting and moving safely around the house
Meals and hydration: preparing familiar dishes, sitting down to eat together and offering drinks through the day
Medication reminders: prompts at the right times (non-medical; our caregivers do not administer medication)
Engagement: music, photo albums, cards, puzzles, simple household tasks and conversation suited to what your loved one can enjoy
Light housekeeping and laundry connected to your loved one's care
Transportation and errands, including rides to appointments, outings and familiar places
Each care plan is built around your loved one's history, habits and what comforts them, and we match caregivers to their personality. Read more about our Alzheimer's and dementia care.
Steps, wandering and sundowning
Steps and stairs: a single-story home can still have a step down into the living room, a raised shower edge or steps out to the patio, and many Westwood and Bernardo Heights homes have full staircases. Our caregivers help your loved one on steps and stairs and at the times of day falls are more likely, such as early morning and at night. At the assessment we can also talk through changes families often make, such as moving the bedroom to the main floor, adding handrails and grab bars, and better lighting in hallways and bathrooms.
Wandering: greenbelts, golf course paths, open space and the trails toward Lake Hodges can draw a restless person outside, and a familiar neighborhood can still become confusing. Our caregivers watch for restlessness and attempts to leave, and gently redirect with a walk together, a snack or a familiar task. Families often add door chimes, motion lights, keep gates and side doors closed and keep car keys out of sight.
Sundowning: late afternoons and evenings can bring agitation and confusion. Our caregivers keep that time calm, with softer light, less noise, a predictable evening routine and reassurance instead of correction. Overnight and 24-hour shifts cover the hours families find hardest.
55+, gated and HOA communities
Many Rancho Bernardo homes are in age-restricted, gated or HOA communities, each with its own rules for visitors, guest parking and use of the recreation center. Our caregivers provide care in these communities the same way they do in any other home. Before care starts we go over gate access, where the caregiver should park, and any community rules for visitors with you, so the first day goes smoothly for your loved one.
When family lives out of town
Many people here moved to Rancho Bernardo to retire, and their children often live in another city or another state. If you are arranging care from a distance, you are welcome to join the free assessment by phone, and we will talk through who in the family should hear from our office and how you'd like to stay in touch with the care team. Alzheimer's San Diego also runs a support group for long-distance caregivers.
Staying home or moving to memory care?
Many families tour memory care communities before calling us. For some people a facility is the right choice. Others do better at home, in their own room, with their own routines and one-on-one time with a caregiver who knows them. At the assessment we'll give you a straightforward view of what home care can and can't do for your loved one.
After a hospital stay
Families often call us when a parent is getting ready to come home from Palomar UC San Diego Health Poway, on Pomerado Road just east of Rancho Bernardo, or another hospital in the area. The hospital team and your loved one's doctors handle the medical side. Our caregivers help with daily life: settling in, meals, bathing and dressing, reminders and company during the day and overnight.
Rancho Bernardo resources for families
A few local places families here turn to:
Ed Brown Center for Active Adults, the senior center on West Bernardo Drive by Rancho Bernardo Community Park (formerly the Joslyn Senior Center). It offers exercise and art classes, social events and trips for older adults, and works with Serving Seniors on a free weekly frozen-meal program for people 60 and over.
Seven Oaks and Oaks North community centers, the clubhouses for residents of each 55+ community, with pools, fitness rooms, clubs and activity calendars.
San Diego Oasis at Rancho Bernardo, a nonprofit center on Bernardo Center Drive with fitness, wellness and lifelong-learning classes for older adults.
Rancho Bernardo Library, the San Diego Public Library branch on Bernardo Center Drive, whose Digital Memory Lab lets you book time to scan old photos and slides or transfer home movies, which can be a good way to build a memory book for your loved one.
Alzheimer's San Diego, a nonprofit offering free support groups for family caregivers led by trained facilitators. It lists a Rancho Bernardo caregiver group on the second and fourth Tuesdays, which is currently full and keeps an interest list, plus an in-person group in Escondido, virtual groups you can join from home, and specialized groups for adult children and long-distance caregivers. Call them to confirm the current schedule.
Support for you and care for your loved one go hand in hand: a group or program helps the family, and care at home helps the person living with dementia.
Respite care in Rancho Bernardo
A spouse caring for a husband or wife with dementia often carries most of the day alone, especially when the children live far away. Respite care gives you planned time away, a few hours, a full day or several nights, while one of our caregivers keeps your loved one's routine steady at home. Families use respite to:
Rest, or sleep through the night while an overnight caregiver is awake in the home
Keep up with their own doctor's appointments, errands, golf game or club activities
Travel, visit the grandchildren or attend a support group meeting
Try out care at home before deciding on longer shifts
Respite visits have the same 4-hour minimum and can be booked once or on a recurring schedule, such as one day a week or every other weekend.
Veterans and their families
Some families here are caring for a veteran or a veteran's surviving spouse. Heritage Senior Care is contracted with the VA for Homemaker and respite care. The VA decides eligibility and costs. We don't file VA claims, but we can connect families with VA-accredited people who do. Learn more about in-home care for veterans.
Long-term care insurance
A long-term care insurance policy may help pay for care at home. Every policy is different, so we offer a long-term care insurance policy review and walk through your parent's policy with you before care starts.
Simple, flexible terms
4-hour visit minimum: time for meaningful help, not a hurried check-in
No long-term contracts: add hours, cut back or pause care as needs change
Free in-home assessment: we meet you and your loved one at home before care starts

No. Our caregivers give medication reminders. They do not administer medication or provide medical or nursing care. They can work alongside a home health or hospice team.
